      The billing code on some live scan forms just means the agency requesting a live scan is going to pay the doj/fbi and rolling fees for the applicant.

      This is not the case for a CCW. The applicant is responsible for all fees, so there is not going to be a billing code on the request for live scan form.

              Those of you who applied by mail- did you leave the witness signature / badge number blank? I'm thinking of submitting in an application since I do have moderate good cause for work

                Yes. Leave those areas blank. That is where the interviewing deputy will sign when you are called for an interview. Please take some time and read through this thread. Lots of good information that will help you along in your process. Good luck.
