He'll just need to put his best guesses for all those items - DOJ has no way to verify any of it, so as long as it's his best estimate he could come up with, DOJ will just have to go with it.
I would put the deceased friend's name, and whatever his address was (or a best estimate of his address), and a best guess for the date, like 7/1/2000.
