However I also know that stuff on forms often find their way into databases.
Databases for medical information are secure and have to comply with HIPPA so there is no worry about the information being leaked or misused.
Until things change that is. A new law, a data breach, whatever. Things are always secure until they are not.
Let that sink in.

The reason why they may become unsecure or misused does not really matter so much as the fact that once it occurs there may or may not be ramifications. What they may be is of course a matter of speculation but they would range from a best cases to worst cases.
So here's what I do.
Do you drink? How much per week? A few drinks per week.
Do you smoke? No.
Do you use recreational drugs? No.
Do you own guns? No.
Now the real answer are more like several or more drinks per week, the rare cigar, no drugs, and hell yeah I own guns, and I carry them too!
My wife and kids have been trained and practice the same.
Remember, declining to answer = YES.
I think the avenue to decline should be done through your elected representatives (at least ones you trust) and without being specific as to which questions.
However you are of course free to do as you see fit.
