Here's my timeline:

5/18/2020 COE applied; 06/22/2020 COE issued - 35 days
5/22 FFL03 charged; 07/02/2020 FFL03 issued - 41 days

Ok guys going to take the jump but not used to the gov forms. Starting with the Live Scan form first because am going to get my prints done for AZ CCW app also.
Top of form:
ORI code ?
Authorized App Type ?
Type of license = 03FFL ?

Applicant Info
Billing # ?

Next section: Your Number OCA number, I assume if this was required for employment ?

Employer Section, again for enity wanting live scan ?

Bottom section for folks doing live scan to fill out ?

I understand that ATI # will be issued and this scan is sent to DOJ.

I asw some links here regardind ORI code but were just live scan form links.

Look at the link in the post above yours for the COE.

The FFL03 is a separate form and requires no fingerprints or photos. Form. Instructions

AZ requires print cards for out-of-state CWP. AZ

The outline I gave in my post are questions for that form. They want an ORI code, I assume this pertains to a reference for what Live Scan is for. All previous answers to this question are links to printable version of Live Scan form.

I am perplexed....when I posted that link, the form was complete; now it's blank.

The submittal process through the CFARS system is far faster than snail mail.

Dvrjon, sir I greatly appreciate your assistance and am in no way a troll. I am earnestly trying to fill forms out properly and have used links previously posted to print out forms, after 5 attemps of printing no form printed with the information pre loading as did your version. I was aware that some of the info on your example did not apply to me, but was only asking if the info pre loaded in the section of question was also only sample info. I deeply apologize for upsetting your apple cart and cutting your thin skin. I appreciate and respect all the insight and help I've recieved in the past on these boards. I made many searches on here and DOJ's site and could not find the answer to my question. How was I to know that the ORI code and other info was auto entered if I was not getting that version.

Doesn't this application process go through CFARS now? Does it not populate the correct form to take to the LiveScan place when you complete the COE application through CFARS? Serious questions, have not tried.

I have the BOF 4008 COE application saved that I downloaded in July 2017 and it has the BCIA 8016 almost completely populated, sans your personal info.

I also had a problem my live scan form was completely blank even using the link provided in the email from doj .would not even allow me to enter it on the form only allowed me to enter my info so I had to write in the doj info by hand

Absolutely no problem sir! I will admit I am severely wounded when it comes to tech savvy and after asking my FFL if he could pull it down and only got a blank form I knew I had to go to higher powers. These boards supply a huge amount of information due to the fact so many of you guys have been there before, that experience and knowledge is greatly appreciated by the majority. We are all in this F'd up state and trying to navigate advancement so that hopefully together we will have a stronger foot hold.

I just got my FFL-03. I submitted them both (03 and COE) on 04/22/20. I received my COE on May 15th. And I just got my 03 on June 15th. Now it's shopping time!
