Actually, UT does.

Only if you are a resident of Utah. Non-resident Utah CCWs are invalid in Florida, Michigan, Colorado, and West Virginia.

You don't need an actual CCW class for Florida. Here is what their "How to Apply" information says:

Not sure because it depends upon what qualifications the instructor has. I did see this post recently:

It's on 10/4 in Modesto. A Utah CCW class, but the instructor will also provide a training certificate good for Florida (as well as a Florida CCW permit application).
