OP, looks and sounds like an ammo issue.

I'd buy some new ammo, maybe 3 or 4 boxes, all different brands, and see what happens. If it was the ammo, that should confirm it.

Were you shooting before this happened? Might have torn the neck off the cartridge and it's still lodged in the chamber. Remove the upper, remove the BCG and look into the chamber with a light. Look for anything that isn't a smooth transition from body to neck of the chamber wall.

Been there, tapped it out with a brass rod. You can also buy a case neck extractor or pay a gunsmith.

Yes the chambers are the no different. The 5.56 freebore allows for higher pressure ammo to be used. Weatherby used lots of freebore for his cartridges.

Then the chamber or ammo is wrong.
A proper 223 wylde chamber should have fit either of those two bullets that were stuck unless the bullets actually measure over 0.2245" in diameter.

A 223 wylde chamber (behind the neck) is actually larger in diameter than a 5.56 chamber.

Sorry but how many rounds total through the barrel? Any buffer / recoil spring mods? Bolt lugs in good shape and extractor functioning? Extended feedramps (m4 ramps)? Any damage to the shoulder of the extracted cases?

Disassemble the upper from the lower, remove bolt and charging handle.

With a dim flashlight inspect the chamber for debris or a burr from machining.

Perform a plunk test by dropping a factory cartridge in a downward facing barrel. Turn the barrel upwards. Does the cartridge fall freely?
Sharpie the bullet black on a cartridge and perforn the same plunk test and lightly tap the inserted cartridge. Turn the barrel upwards. Does the bullet fall freely? If the cartridge is stuck, mark the case at the ejection port so that you can locate any possible issue with the barrel. If stuck, point the disassembled upper in a safe direction, slide a cleaning rod from the muzzle end. Always take extreme caution not to damage or contact the barrel crown (this is behind the flashhider, the very edge of the barrel's rifling) The bullet will just need a slight tap from the rod to release.

If the barrel checks good, you'll need to have the bolt, bolt lockup and loading cycle inspected.

15-20 rounds is not much, but it's far better than if the problem happened on the very first 2 rounds fired.
This makes it more like to be an ammo problem than a chamber problem unless the first 15-20 rounds were different ammo than the two problem rounds.

If this is factory ammo both xm193 and 855 COAL and Ogive should be more than a blonde one off the lands of a .223 wyld chamber/throat. Photo 1 and 2 show cartridges that are crimped high on the cannelure. It could be the ammo but the case dims would need to be pretty far out. This is like diagnosing an engine noise for someone over the phone lol.

Bear creek took the upper back to swap out the barrels. They said that it shouldnt have happened and are going to replace and test fire. The ammo has run without issue in 4 different psa uppers. I guessing the bear creek upper had to be out of spec. Ill run it when i get it back to rule out the ammo.
