Yes, you have to remove the rear sight leaf because it becomes a pivot pin location for the TWS Dog Leg 2. You have a few options though 1) Get a low profile picatinny aperture rear sight (several available) that puts the aperture close to your eye in the manner of most modern Western arms. A good set of sights can transform an otherwise mediocre rifle. TWS makes their own milled fixed aperture rear sight assuming you are using an adjustable front sight with adjustments for elevation and windage (as virtually all AKs have) to a fixed zero range. It's not great loss to lose the tangent leaf sight, most folks have it set to the "battle zero" and leave it alone anyway. This is the sight I bought to replace the TWS low profile sight on my Yugo M72 with TWS Dog Leg 2.

I prefer to do my adjustments from the rear sight, so I'll center the front sight and lock it down with threadlock and do everything I need from the rear.

I like this option better than rear peep, as I would only use irons as backup anyway... I'm waiting for production release then I'm all over it!

if this rifle was made with a side rail mount by russia, then use the side rail mount. there is a reason that it originally came with the mount on the side.
