My money says a heavy charge of Bullseye (or 231) got that one. This reply may belong in the reload section of the forum, but this may be another illustration of why I am so adamant that beginners should not use progressive loading machines until they fully grasp and have some experience in actually learning about the process. To learn that process, some time using single stage equipment and how to deal with problems is what it takes. The fact that a valuable firearm was destroyed only makes it more so.

I agree NOT starting out with a progressive press. I began with a Lee Loader, where everything is manual and the only press you need is a rawhide mallet. Powder charges are measured with a tiny yellow plastic spoon.

Progressed to a pawn shop RCBS single stage press...after I had a feel for what I was doing.

I also cannot emphasize enough as to the needs for focus and lack of distractions while you reload...that includes kids running around and the TV.

Shooting is very similar - many people who have never done it are extremely nervous about "doing something stupid" until they see how the safety is handled.

Reloading is a bit trickier as it's possible to miss things, but it's normal to be apprehensive. Attention to detail, checklists and understanding of the process are a must.

I'd say either a mismanaged changeover or a double charge. I'd respectfully posit that a double charge is most likely, and easier to do with a single stage press.

The downside of a progressive for beginners IMO isn't the likelihood of a mistake, it's that getting it set up can be frustrating. Virtually any operational mistake you are likely to make in a progressive can be done in a single stage; the progressive just lets you make it many times before you notice. Additionally a SS allows mistakes like a double charge to be more readily made.

In any case, too bad it wasn't a Ruger or something.

I've been reloading/handloading for over 30 years. I find it to be extremely safe. I've never had an issue like that. Maybe the person that reloaded it asked his ex-wife to fired that gun. If could happen. Risk? - just pay attention to what you are doing.

Why would I spend triple the cost to have fun in this hobby and pay for factory ammunition? I guess for those who don't use caution need to buy factory.

It's obvious that clear engineering design principles and processes were not used when reloading for this gun. If only the owner was highly regarded in his field with multiple engineering degrees, this would not have happened.
