Here's a nice animation of a G19 firing:
Note at second 5-6 that the lower edge of the fired case rubs across the top of the loaded round in the magazine.
If the lower edge of the extracting case snags on the case mouth of the loaded round in the mag, the fired case can be bumped out of the extractor or at least get twisted out of place and will not be sitting square on the breech face.
The case needs to be sitting square on the breech face when the slide comes back far enough for the fired case to hit the ejector.
If not, the case will not get a strong hit from the ejector, and won't pivot smoothly around the extractor tip and out of the breech area.
The various posts on erratic ejection lead me to believe that this could be the problem.
There are a couple ways to fix this problem.
One:
Slightly bend down the front of the mag feed lips so the top round in the mag is sitting lower at the front.
A little bit goes a long way here and you do not want the feeding round to nosedive, so don't bend much.
I know the Glock mags are tough and may not bend so easily, so consider the next option.
Two:
Slightly lower the ride height of the mag itself.
This can be done by either taking a little material off the top of the ledge on the mag catch or by slightly raising the upper edge of the catch hole slot in the magazine.
Again the Glock mags are tough so they are not easy to file.
Given the low cost of a new mag catch, and since they are made of polymer and easily modified, I would get an extra catch and file it a bit to get a lower ride height.
And no, I am not picking on Glocks here, this problem and fix is the same for all autoloaders.

Newer Glocks have to be broken in first with high quality brass cased ammo 115 or preferred 124grain. Older Glocks ran almost perfect out of the box. Glock has redesigned the port ejection, gone to MIM parts and has tweaked the extractor angles which has caused issues.Comment

I'm sorry. I should have specified that I was thinking it maybe have been due to the type of casing. I like the price of steel case but reliability of brass

Another variable on the problem is that some ammo has thicker cases and less crimp at the case mouth, making the possibility of snagging the case mouth on extraction more likely.

One day i had 4 Glocks having FTE. It was a bad batch of ammo. My friend returned the ammo and store was aware and replaced the ammo.
You used 3 types of ammo, it could have been anyone of them.Comment

What brand was it? I usually buy at Walmart. They stock Winchester. I've never had any issues.

it happen to all brands of ammo. i thought it was the ammo at first then tried a different brand and it happen again...i had a light mounted thinking it was the cause and it still happened.Formally known as el Chingon...Comment

i was just doing some research on this. is it a gen 4? if so in the gen 4 the ejector pin was causing issues along with their new recoil spring they first put in the gen 4's, there has been issues with them. they have a ejector and spring replacement program that if you call them they will send you a new spring...if that does not work they will replace the ejector mechanism. last i heard it was free of charge... hope this helpedComment
