    I could be wrong, but I don't think your going to find a form to do that. It is probably going to involve a transfer at a FFL, along with a letter from the chief.

      Transfer must be done at a CA DOJ "Assault Weapon" dealer. Not every dealer in CA has the ability to do a transfer of an assault weapon. You will basically need everything to purchase a assault weapon from a dealer, the agency purchase letter.

        Its a blue card..The officer will need to go to Proforce to get another..they just don't hand them out..
        It is not available online..

              "Blue cards" have not been in use for 10plus years.... When a transfer is done at a CA FFL with assault weapon lincense, the FFL dealer will fill out a Assault Weapon Registration forum via Adobe forum.... There is no way to transfer with out a AW letter from the department..... Just not possible
