They really have though. Either I didn't apply enough force (twice) or I just suck at swaging. Maybe both.

OP, I have been using a Dillon 650 for many years, and I have repeatedly experienced the same problem of primers being seated upside down and side ways. I have bolted my press to a stand, which allows the press to move a small degree while operating the handle. I believe if I bolt my press to a more stable platform, I will eliminate the primers from flipping upside down and sideways. The third primer looks like your primer pocket was too small, and as others have commented, you may not have completely removed the crimp from the military case.

          it happens...

Yes this happens!! The gremlin was crimped brass and movement of the press during the reloading cycle. You know what to do about the brass, make sure that your reloading bench is secured to the wall studs and floor. My bench is a solid core 2" door on top of 2x4 framing screwed into the wall studs and the 4x4 leg is screwed into the floor.

That is likely a contributing factor. I built my reloading bench; weighs about 100lbs but I haven't anchored it so it does move a bit during the process. I'll give that a try!

As I said the movement of the press while operating the press is the reason for these malfunctions. In my case, I only get a couple of these malfunctions, which is not concerning unless I was loading ammo for self defense.
