Bullet coming apart inside chamber?

Im assuming the bullet just fell out the business end.

Bullets just don't fall out of the end. If the bullet did just fall out then you were given loads with bullets that were not sized correctly. A barrel gets smaller the further you get to the end.

The most likley reason why this happened is when it was chambered the bullet made contact with the rifiling. The rifling held the bullet as you removed the round. The bullet must have fallen out when you were inspecting the problem. My guess is the reloads you are shooting are loaded a bit too long.
