There was a report a couple of years ago of a cop who had a "kaboom" due to overpressure.
He unloaded every evening when he got home and re-chambered the same round the next morning... supposedly.
He estimated that he had rechambered the same round something like 300 times.

I find part of it hard to believe... but understanding that the details in the story may not be 100% present or correct... Picking a nit, why would he rechamber the same round every morning? Unless he doesn't top off his magazine, at the most it should be every other time... unless he manually chambered without the mag and then inserted it (in which case I'd expect the extractor or case rim to be pretty chewed up).

But either way... this means that he went at least 2, and possibly 4 years with the same round topping the stack.
Cops qualify more often than that... unless he qualified with FMJ and he carried the same JHP for 2-4 years.

And finally, what were the conditions of the "kaboom".. I can't imagine the round self-igniting from setback. I've encountered "Independence" 45 that had so much setback that I left them on my desk as a reminder to inspect EVERY round before it goes into the mag... I think this was shipping damage. One round had so much setback that I would imagine that the case would hang up on many guns.

So that implies that he fired it and it blew... but if he qualified with different ammo, why shoot that round after 2-4 years... no mention was made of why it was fired.

I allow 4 times, I put a black marker line every 90 degrees on the head for each time I put it back in.
I do not let the slide down full
speed ahead so I doubt I will ever have an issue.
I made a dummy reload once and let the slide go on it 30 times in a 1911, the COL remained unchanged. I have seen one or two move before but not enough to worry me.
I think primer damage is the bigger issue. No noise from a defensive load is always no good.
