Didn’t they cover this in hunters education class?

Don’t mix shells in your pockets.

Besides, it only matters when you manage to stick a live 20 gauge shell 3/4 down your 12 gauge barrel, THEN fire off a 12 gauge shell - but that would take a series of failures, and require posting on YouTube.

          You might own:

          1. a 12 ga. for ducks and/or a police pump for home defense
          2. a 20 ga. for doves

          I definitely wouldn't carry both a 12 and a 20 on the same hunt:

I would only carry a 20 ga. gun on dove, pheasant and grouse hunts and only a 12 ga. on duck hunts.
I keep the yellow shells out of the 12 gauge. Easy enough.

Oh, now I get it: yellow is for the cowardly weak-shooting 20 and red is for the bold and hot-and-spicy-like-taco-sauce 12!!

Yeah, I guess that the manufacturers didn't wanna stretch it too far and make every gauge/bore a different color. Other gauges/bore will also fit into other larger gauge/bore barrels.
Why did they stop at 20/12? Probably because those are the most popular gauges.

I was at the Last Shots Fired event at Vado Del Rio (Camp Pendleton) over the weekend and someone broke out some old 20 ga. AA shells. They were red. Probably from the 70's or 80's.

Are you actually hunting birds? Or is this just another what-if question?

Not at all the same. .40 in a ,45 just proves you're stupid. 20 in a 12 can kill you and will, for sure, ruin your firearm and your day.

People drop a 20 into a 12 and the shell falls down the barrel.
It's when they drop the 12 in the barrel without first removing the 20 that causes all the problems.
I think valley of the moon trap club had a barrel on display years ago of one.
Guy used the same vest without checking and fed the gun the wrong ammo.
