it is pretty obvious that your top rail is not parallel with the barrel.
Possible reasons for this is that the ultimak was machined out of spec (Possible), the top rail is mounted improperly (probable), or that the receiver/barrel relationship is off (not at all likely).
You may want to see if another scope/mount works well and is able to zero somewhere near the middle of the scope's adjustment range.
If the same conditions happen with the new scope, then it is the rail that is the culprit.
