        Very nicely done!
Clean and professional looking and it covers all the bases.

This is what I do. I have the page in MS Word, with one entry per page. The info goes in a table at the bottom and I put several photos above. I add a sequential page number, print it out and add it to my binder.
At gun gatherings I pass the book around and everybody enjoys looking at the pictures.
Then I pull out the photos of my grandson...

Is it possible to get this form in excel format, or a PDF that can be edited? I'd like to fill in the data on my computer and it doesn't seem to be one of the PDF types that allow you to edit it online.

I collect Military Arms and enjoy shooting in local matches. I also collect older Lever Actions, especially those chambered in odd/old cartridges. If you have a nice old Winchester or Marlin in 25-20, 32-40, 38-55, 40-60, 45-70, etc etc, please PM me and we can work out a deal.

There are programs out there to unlock the PDF but I would think it would be easier and faster for the OP to do it if he is up to it.

Hey all... I created a new version of the form. What I did was seperated the Acquistion and Disposition data. This if for persons who want to edit the PDF and print single sheets out on a as need basis.

I dont think that it is a good idea to enter multiple firearms on one page and print the page out after the last record is completed, therefore I wont produce a sheet for this. I dont think anyone is doing this but if you are then its your choice.

                    the pdfs contain the following:
                    *tool tips for each field
*pull-down for action type (with user entry as well)
                    *notes field for s&g's
*entry masking for FFL ID # field
                    *date formatting

There is also a disclaimer on it... pretty much use at your own risk. The disclaimer on the form will not print out... didnt want to ugly it up

                    Enjoy.

Shouldn't the Acquisition and Disposition be on the same page? Otherwise you end up with two pages for the same gun. And looking at the 1st page (Acquisition) you can't tell that you no longer own the gun.
Also, the two pages probably won't be next to each other because the pages need to be numbered and you may have more "acquisitions" before you dispose of the gun.
The sample format on the BATFE website shows both on the same page.

The important part is that you have a record of an acquisition and a record for a disposition. I suppose once you dispose of the firearm you can hand write in the notes of the acquisition that is is 'disposed'.

If I am understanding this correctly, you have decided to use a separate page to record acquisition and disposition. If this is correct, then I would suggest that you take a look at ATF's required format. The record is specifically designed so that the record for the firearms acquisition and disposition are together on one form. There is an example of the required format here.

The important part is that you follow the law and use the format approved by ATF. If you want to use a different format, you must obtain approval from ATF's Director of Industry Operations prior to it's use.

I would suggest that all 03FFL's use a commercial printed bound book and log everything by hand. This will keep you out of trouble if you are ever audited.
If you are audited and there is a question about whether or not your books have been altered, a computer printed bound book now means that your computer will be involved in the investigation.
You can always keep a computer printed record to supplement your hand written bound book if you want something to refer to that "looks nice", but your hand written bound book is your official record as required by law, and the only record that you are required to show if audited.
