I had this happen to my M&P sheild once teaching my wife how to field strip / clean and reassemble. The Gide rod was not placed in the center and when racking the slide it jumped off. What made matters worse is we inserted the mag in order to lock the slide back due to her not having enough strength to pull it back and use the slide lock.

At this point the gun would not lock back and the takedown lever inside the gun was pushed back from the mag being inserted. After watching the wife freak out thinking she just broke my brand new shield it took me 3 min in a vice. Claped down the slide pushed down on the grip and released the take down lever.

Once that was complete the slide came right off. Hope that helps.

In your Safety and Instruction Manual (provided you have one) see page 24, figures 33 and 34.

Not at all. It is a reassembly flaw

most likely a reassembly flaw on my part. probably didn't align the spring guide and it popped off to the side when the slide was racked.

the recoil spring is quite lumpy though and i'll try straightening it out per fubar's latest suggestion.

I had a similar thing happen to my friends XD, turned out the recoil spring and guide rod broke completely.. but I'm glad nothing is wrong with your buddies Shield.

Yeah, it's something they didn't get quite right. There have been several reports of the guide rod being too picky on exactly how it's positioned. But this is the only one I've seen where it popped out during use.

I've locked up my Shield in almost the same way once after putting it toghether, although it wasn't so difficult to get the slide off.

I've seen this happen on a Glock, also.
It can happen if the recoil spring assembly is misaligned.

Handle of the spring should be seated as far down as possible against the barrel. There is no clear cut out for it to rest on so just check for that and that it is parallel to the barrel.

There are videos on this that I have saved to my favorites. But I have over 1500 favorites. Sorry.

It seems to be a common problem where assembler sets the spring off to an angle- at least with the XD and the Beretta (it happened to me) and as in your case.

I don't believe it happened during use, I believe it was a reassembly issue after disassembly and cleaning. The original description wasn't clear.

You can still release the slide. Pull the slide back, lower the takedown lever on the side, insert an empty magazine, let the slide move back to the front, pull the trigger, release the mag. The slide should now be able to be pulled all the way forward and removed.

OP, did this happen while shooting or after cleaning/reassembly?

3 people have already nailed it - guide rod/spring misaligned while putting the slide back on. OP- were you able to fix it or do you need more help?

Happened after cleaning and re-assembly. That's why I am leaning towards re-assembly error on my part.
