    I've made sure that's not the case, even installed the extended release for a bit to make sure I wasn't touching it at all. Could it be too light of a bullet causing the slide to not go back far enough? Was happening with 115 grain.

      To heavy a spring will cause it to not go back far enough to lock back.

        Still have the stock spring in.
        Tried some 124's today, went through about 250 rounds without a problem. So I think it was just the ammo I was using.

              There's not a semi-auto pistol I own that doesn't do that. I've simply learned to accommodate that possibility and don't even hesitate to reload and continue shooting regardless. I'm the same speed in competition whether the slide locks back or not on my emergency reloads.

              The way I look at it is it's good tactical practice for if that happens when the chips are down.
