Im going to say ammo. It may have been just a little under power.

One FTE does not constitute a problem with any firearm.

Keep it simple. Ammo? Mags? Lubed correctly? Recent mods?

If it habitually gags then look to other corrective actions. One at a time.

Can't tell from that picture. If you push on the extractor near the FPS does it rotate at all?

Yes, it rotates. Actually I tried another two 1911 (RIA and SA Milspec) and found it rotates in them too.

It happened twice for maybe 150~200 rd in total. Is it possible because of my limp wrist? Long time ago I have some issue with my new g17 but not any more.

Also, if you are not sure if you are limp wristing or not, definitely don't spend any time screwing around with the gun.

That's an issue. You can fix it by fitting an oversized firing pin stop. They cost about $15 from Brownells.
