Help with CA COE livescan form

So I'm trying to get a livescan form done, and the operator I went to said that alot of the information needs to be filled out but I can't find the information online.

Examples would be the ORI number, authorized applicant type, mail code etc..

Any help would be awesome, thanks

The data is there you just can’t see it. This is because of the way DOJ constructed the file and some compatibility issues with some PDF viewers. There are tips for how to see it, but if all else fails use the link I gave up top
