Need Info on transferring AR to my Son

My son is 23 and I am giving him 1 of my ARs. We want to transfer it into his name. I got this rifle after Jan 2014 Law went into effect so DOJ got info.
I know about the form and $19.00 to send. But in looking at the Form you only list 1 persons info. Who's info do they want? I am guessing it would be my Son's info and in the small box he acquired it from Father. I find it strange this form does not ask for more info on Current Owner.
Is this correct?
Also does he need an FSC before the transfer?

Also, your son should be the person filling out the form and mailing it with payment to CA DOJ.
