Having trouble with which Live Scan PDF to use for COE

In the COE Application form it mentions under the instructions section to use Live Scan Service form (BCIA 8016). I found under the DOJ website in the firearms form section there are currently two 8016 forms.
Which form is to be used, I have had not much luck with this determination. Any information would be greatly appreciated. I originally thought to use the most current form (first Life Scan link Pdf), but wasn't sure.

thank you very much... Do I have to mention to the Live Scan tech that fingerprint impressions are required to submitted to DOJ and the FBI? I would imagine they should know but I do not want to assume.

If you look at the top of the form it tells the live scan operator all they need. You'll end up with a copy and application number you put on your coe application.
