First pass at a ammo log. . . comments welcome

Here is my first pass at an ammo log. What do you think?

I figure one sheet per customer and I'll just keep them in a three ring binder. (Since this is a three ring circus)

    Let's see the regs.

Commencing February 1, 2011, a vendor shall not sell or otherwise transfer ownership of any handgun ammunition without, at the time of delivery, legibly recording the following information:
    (A) The date of the sale or other transaction.
    (B) The purchaser’s or transferee’s driver’s license or other identification number and the state in which it was issued.
    (C) The brand, type, and amount of ammunition sold or otherwise transferred.
    Ch. 628 — 2 —
    (D) The purchaser’s or transferee’s signature.
    (E) The name of the salesperson who processed the sale or other transaction.
    (F) The right thumbprint of the purchaser or transferee on the above form.
    (G) The purchaser’s or transferee’s full residential address and telephone number.
    (H) The purchaser’s or transferee’s date of birth.
I was making up a form for my store to comply with this silly law and noticed (I think) they forgot to ask for the buyers name. I'm no lawyer, far from it so I'll let you guys tear me apart now.
It it wasn't for the stupid thumbprint, I would just keep it all on my POS software. I wonder if there is an easy way to scan a thumbprint and stick it on a sales receipt electronically in Quickbooks POS?

Hooray for our thorough lawmakers! So anyone is just a scribble and a smudge away from an ammo purchase?

No actual experience, but USB fingerprint readers (1 example) are not expensive.

I have looked into this and found it is possible - extra software required --means extra cost -- lots of cost

That should work. You gonna set up a dedicated printer/scanner to get the DL printed on the reverse so you don't accidentally print on those sheets?

I did not see a requirement to get a copy of the ID. So why do it?

Also, the law appears to be written loose enough to allow a log to be created per person with multiple purchases on one page. That is why I've got multiple lines on my version above. The way you have it written up if a person buys three type of ammo you will need to get three fingerprints copies of ID's etc.

I want to make this *legal* and as easy as possible on the purchaser.

Well the copy is for me...how can I prove they showed me valid ID?

I'm trying to have to have the customer write out as little as possible. The ID number/date of birth/address is on the ID, so they wouldn't have to write it out with the copy of ID. In the case of Military ID, I'll just have them write an address below the ID.

as for multiple calibers in one purchase, I guess I could make the lines long enough to list multiple brands/calibers/QTY. They'd still only need one copy of ID and one finger print as long as all the ammo is listed on the same page.

Again, I don't see a requirement in the law for you to prove this at a later date. Come on, there are all kinds of people out there with fake ID's and SSN working for legitimate employers. Are we really expected to spot a good fake? I doubt it.

I don't need to "spot a good fake"...For my own records, and any future questions, I want to be able to point at something and say I did my due diligence is all.

After looking at your log again I have a question. Are you making a sheet for each individual customer, and then whenever that customer comes back in you just enter the new purchase? What about if the information (address) changes? Then you make a second log? We will have to store the record for 5 years as well...so if a customer only purchases through you maybe once or twice a year, the log would be taking up space in your binder continuously?

Yes, one sheet per customer and I plan on keeping the log until it's full or there is a change in information.
Binder space I will deal with if it becomes an issue. I can see scanning and storing at some point unless they make us keep it on paper.

I have a Tablet PC that accepts signatures and has a fingerprint reader I may play with and do the whole thing electronically.
