If that's how the gun came into your possession...then yeah

It's better to put "Intra-familial Gift". That box is to tell them if you are using the Op-Law exemption or the Intra-familial exemption. Some DOJ employees have had trouble figuring that out, even when the form says "John Smith (my father) in one box and "gift" in the other box.

That's good to know. One Calgunner got a call from CalDOJ because the clerk couldn't figure out what kind of transfer had taken place, even though he had written "gift" in one box and "from father" in the other box.
