Would like help with Live Scan information

Hello everyone, I am attempting to get my FFL03 and COE. The FFL03 application I understand and have completed. The online instructions state that in order to acquire I need to get a Live Scan done and that I should go to a Live Scan center with a pre-filled Live Scan application. I found form BCIA 8016 online but it is a blank form. I get that I am to complete it by adding my personal information, what about the rest of the form? What information should I enter or do I leave the rest of it blank? Do I notify the Live Scan personnel that I am applying for a COE and they then fill out the rest of the form?
