FFL "Supplemental Information on Water Quality"

I am apply for my 07 FFl and have no idea what to put for box 1 and 3 where it says Form Number and Serial Number. I can't for the life of me figure out how either of these are suppose to be answered since the Form Number is on the top of the document, and serial number has nothing to do with a firearm.

Interesting.. I have an 07 and never had filled out that form. Granted it was back in 2008/2009 when I got it.

Yes it is this one. They also sent me an environmental impact form which was pretty straight forward, but this appears to be a supplemental to the environmental impact form?

I called the Portland BATFE office for more info. Hopefully they call me back soon, but I would like to get my form in asap.

Do not send in either form unless you intend to discharge wastewater into a navigable waterway. I am not somewhere where I can "you chapter and verse right now as I am mobile will be able to later on today

I got a hold of someone at my local ATF office and he said to fill it out to the best of my ability and we will fo from there. My shop will be within a 1/4 mile of the Williamette. I don't think some metal shavings will make it into the river though.
