I read the post, but you missed the other thread and the actual issues. You should have replied to the existing thread, but I guess you did not notice it as perhaps you were not paying attention.
It is NOT illegal for them to not include the form which you can download and print and include. If you think it is illegal for them to not include the form, please provide the CA PC which was violated or other law which they have violated. They included the form to be nice in the past, but it certainly was not required.
It is annoying, but some received the form, others did not.
If you want to attack the CA DOJ, there are many valid things to attack them on, so please don't attack them on a bogus basis.Kemasa.
