What model is this and does it hold zero? I know its used but is the glass scratched or cracked anyplace? Thanx

I don't remember what model it is. I found it in my closet and forgot I had it. It used to be on my bolt action .22LR and held zero with this rifle. The glass is not scratched or cracked in any other place.
