I think you need to drill it out and start over. What most likely happened was as you were setting the rivet the receiver rolled on its side twisting and causing your problem.
My suggestion is to shim your receiver o the jig for the next set of rivets. Get as much support under the receiver as you can. This will prevent the receiver from rolling over and twisting -
start over. its a gun. your face will be right there if something goes wrong.Been gone too long. It's been 15 to 20 years since i had to shelf my guns. Those early years sucked.

Just make sure all your holes line up. Dermal them out if they don't. A little misalignment becomes a big problem if the rivets try to straighten every thing out. The trunnion is a more rigid piece and if the receiver holes don't line up with the trunnion three diver will flex, twist, bend to line up right. Happened to me on a tapco flat. Had to cut it up and start over.

Those blocks and those buck "buttons" suck balls IMO. What you need to do is make your own buck bars, I made mine out of 1.250" round stock. I used a 7mm ball end mill to make the cup and turned the profile so the receiver sits just above the flat of the bar. Also, get some HDPE from Tap plastic and make a plate that supports the entire length of the receiver. I'm around a 98% perfect rear rivet rate, but I can also fix it as long as I catch it first.
Also, make sure to tighten that guide down really good, and that you can spin the alignment ram freely. If there's any resistance, it's not straight. Take the time and do it right the first time, if not....well you'll take the time to do it the second or third time correctly.There are 3 kinds of people in this world.
