Does anyone make custom painted/engraved Dust Covers?

It would be odd to pay for premium hand-cut engraving of the type you find on fine sporting pieces given that this is an AK, but it can be done. It's usually priced by the inch, by complexity or both. Laser engraving is cheaper and depending on the equipment can even compensate for the radius if you want the engraving to wrap around the curve. There are plenty of places that do that kind of engraving. Engraving on the receiver generally requires an FFL license, look for places that specialize for a slightly higher price. A nice bit of engraving by hand could set you back $250-500 depending on size and complexity.
As for painting - what did you have in mind? If it's a couple coats of a specific color, you could probably handle that yourself. If you want some ornate scene painted on top of, or instead of engraving and then protected with a thick clear-coat, you may have some trouble finding someone to do that and I have no idea on pricing for that.
