Load data tracking log/spread sheet

Does anybody know where a printed log book or sheets, designed for tracking loads and builds, might be sold?

If not, any recommendations/hindsight on what columns a spreadsheet should include?

Aside from all the component data, I include chrono'd speed, Overall Length/length off lands, Group size (you can break that down in a number of ways) and Group sequence (ie; 2 of 10).

Excel or Open Office Calc. are fine. Don't get lost in the details, the biggest variable is behind the gun.
