need help finding 06 FFL form

need help finding 06 FFL form

looking on the DOJ Form ordering website and I can't find the one for an 06 FFL.

all I can find is:

ATF F 5330.3A (Form 6, Part I) — Application and Permit for Importation of Firearms, Ammunition and Implements of War
◦Common ATF Form 6 Errors
ATF F 5330.3B (Form 6, Part II) — Application and Permit for Importation of Firearms, Ammunition and Implements of War (for use by members of the US Armed Forces)
ATF F 5330.3C (ATF E Form 6A) — Release and Receipt of Imported Firearms, Ammunition and Implements of War
ATF F 5330.3D (Form 6 NIA) — Application and Permit for Temporary Importation of Firearms and Ammunition by Nonimmigrant Aliens

can anyone point me in the right direction? am i just looking at the wrong website?? This is where I'm ordering them from

the form number does not equal the type of FFL
