                Poor equipment maintenance and inspection. Holster obviously had safety issues long before this. You should inspect and maintain your equipment on an ongoing basis to prevent failure.

                This really is no different than an airplane crashing because nobody checked the oil.

                  This has nothing to do with a lack of grip safety, and everything to do with using crappy worn out equipment.

                    There's already an echo in this thread but I'll keep it going.

                    This is not a case of external safeties being better, but rather worn out equipment and improper inspection is so, so much worse.

                    Yet another reason, for me, to love retention holsters (passive or active). Nothing like that *click* to give another level of assurance your firearms is secured.

                      That old leather holster had been like that for a long time it seems, the user was just too inattentive and lazy to fix the issue. Leather doesn't just fold like that instantly.

                        Grip safeties are needed by those who shouldnt be carrying firearms. A proper holster would have prevented the issue.

                          His holster was not maintained in proper condition. It is necessary to check all of your equipment for serviceability not just your firearm. I want my Glock trigger fully enclosed and covered by the holster.
