Yeah, it happened to me. I was reloading with a Dillon auto indexing press and just couldn't wrap my mind around the automation. I had to check every tenth round and forgot to replace the powder. The primer put the bullet about half way down the barrel of a 29-2 w/five inch barrel.

I like to never got that thing out. They say use a wooden dowel to pound it out. Wrong. The dowel splinters.

I hadda cut off a section of a cleaning rod to get it out.

Happened to me with factory Privi .380. it had bad powder. Bullet lodged halfway in the barrel probably by primer force alone. The powder was blasted all over the inside of the pistol. The powder definitely looked bad as it was slightyly green tinted and looked like the grains were swollen compared to the powder that i pulled out of another cartridge to compare.

By chance it was the last round of the magazine but i surely could tell something was amiss. Was able to easily tap the bullet out with a wood dowel.

Happened with a .22lr to me once. Fortunately I recognized the issue, knocked it out with a cleaning rod, and just went back to shooting.

Never had a squib, but I did have a FTF with reloads I bought at a gun show. Guy who reloaded put the primer in upside down. No more gun show reloads for me.

Next round fired bulged the barrel. Luckily for me, the squib was lodged at the end of the barrel and the next round pushed it out but the damage was done. Gun sent to Winchester as well as 3 boxes of ammo from the same lot. They sent the gun to Glock for inspection and repair, but it's been over a month that I haven't had my every day carry gun. Just glad there was no injury.
My Glock 22 has never liked the winchester ammo. At first, I thought my glock was the problem. I called up Glock and they told me to switch to either, Speer, Federal, or American Eagle. Since then I have never had another issue.

My reloads. First one in thousands of reloads done. The cause was an incomplete powder burn. Not sure if it was a bad primer, or if moisture got in.

The 1911 didn't even cycle the round. When the hammer fell it just made a little "puff" and the projectile was lodged about 3/4" inside the barrel.

It happened with my first batch of reloads. I wasn't seating the primers deep enough. Luckily, the bullet lodged shallow enough that the next round wouldn't chamber.

ETA: This is why I'm paranoid with my reloads and wont let anyone else shoot them.

I've seen it happen OFTEN with winchester white box (NATO loads). In most cases you should realize the issue when it happens EXCEPT when you're shooting full auto and it's too late to know. This happened to us when we were out shooting a select fire MP5, a transferable one mind you. It caused an over pressure explosion and blew it up. Ruined a $20,000 weapon. All of us now do not touch white box 9mm ever again, there were 4 other squibs from different lots just on our range in the last 18 months.

First time I did 38 spl it was in a progressive lee. Got a squib every 10 rounds. Had to pull about 300 of them with the inertia hammer. That sucked. It was probably the way it was set up but after that went back to single stage and shine a light down the case for powder.
