Initial fire / Officer-Involved-Shooting Jam Frequency Question & Reliability Issues

I'm an avid follower of PoliceActivity and any OIS / bodycam shooting channels. I noticed in a surprisingly large number of videos, the officer's handgun jams, misfeeds, or just has some kind of malfunction in a critical moment.

It doesn't seem like it's model specific, probably just because Glocks are the most common department issue amongst all the videos.

I'm trying to discern the major causes / factors for these malfunctions:
    • Is it an initial limp wrist because the officer is trying to get rounds ASAP on target, therefore their grip isn't optimal/ideal?
    • I see most of the videos is a failure to feed, or a failure to eject. I doubt these department handguns are ill-maintained..?

I feel like these shootings, videos, experiences can all provide some sort of realistic insight to problems that can arise. I imagine the worst nightmare is when your department issued handgun has a stovepipe and the perp is about to shoulder his AR-15 at you.

What's your opinion on the causes of most of these malfunctions?

Any LEO/officers who can chime in are greatly appreciated as well.

      IMO...
Most firearm stoppages for LEOs are a result of complacency; which affects their firearm handling, firearm maintenance, and firearm accuracy.

Depending on the LE agency, they tend to not hire "gun people" and prefer to hire applicants that have no firearm experience.
So, most LEOs tend to not to be "gun people" and only view their duty firearms as tools that are associated with the job.
Overtime, those LEOs tend to not want to practice or maintain their duty firearms while off-duty and will only train when required to do so (once or twice a year).
This results in poorly maintained duty firearms that cause stoppages when used on the streets.
The reasoning for the lack of proactive training/maintenance is that the LEOs start associating it (training/maintenance) as part of the job and they not wanting to deal with the job while off-duty due to burn out or complacency.

To counteract this, large well funded LE agencies will often employ an economic incentive to motivate their LEOs to be proactive with training/maintenance while off-duty.
However, smaller less funded or defunded LE agencies do not do this due to budgetary constraints.
