        +1 to Offrdmania's post.

        We had a very similar incident occur at Camp Pendleton. The magazine blew out when the first round was fired. The investigation found that the weapon had been improperly cleaned after its last use. The bolt cam retaining pin was not replaced after cleaning. When the bolt moved forward, there was nothing causing it to to turn and engage the locking lugs. The net effect was that the M16 was turned into a direct blow-back weapon.

        Fortunately, no one was hurt.

            It seems like he was having bad primer problems than just before it blows he says at 1:00 I did not load these Brian did! OOPS!!!!!!!!

              He has several that don't go off and complains about bad primers. Then asks "Who loaded these?"

              I think after one or 2 of those "bad primers" I would have stopped and dumped that load of ammo for further investigation.

              There's clearly a problem of some sort, but dude doesn't stop until one finally blows up....

                  I'm guessing squib, stuck in the throat. Possibly from a round with no powder charge. That would explain why the next round wouldn't chamber. He bangs the assist until it's shoved into the pipe enough to chamber. Says a lot about the design of the rifle that the blast was directed away from the shooter.

                  This is why, if something seems not quite right, STOP and check!. My buddy blew up his Makarov due to a squib and the same buddy stopped me in time from blowing up my .30-'06.

                    I've heard that you should never use the forward assist. If there's something wrong, forcing it probably isn't gonna make it better.
