You need to take it to a notary, most ups store will do it for $15

What is that form for, and when is it used/required? Having just bought a new revolver, nothing was said about a form needing a notarized signature. I have my handgun 'license', and was told to bring my ID and a utility bill/car reg for address confirmation, but nothing more.
Can anyone shed a light on the linked form?
        Thanks!

OP is just checking to see if he can legaly own a gun. To do that you need to have a form notarized stating you are who you say you are. This has nothing to do with purchasing a gun
