      Have you ordered one within the past two years? The notarized form from previous orders is good for 2 years.

          I think they'll give you the opportunity to correct it, they move at a snails pace so you'll have plenty of time. Or you could be proactive and get that page notarized and send it in.

            I did the same thing just a few weeks ago. Received a polite e-mail alerting me of my over-site and said they would hold the order until I could send the properly notarized form back. Done and order is in, very nice to deal with.
