Intra-Familial Firearm Transaction letter??

My father lives out of state and he is going to gift me a old revolver he has.
My understanding is I have to find a FFL that would except a shipment from him. I was told the firearm has to be shipped with a copy of his drivers license AND letter stating he is giving it to me as a gift. My question is does anyone have a sample letter they have sent that was excepted?? I just do not want to miss anything in the letter.

Have your father write your full name and address in the letter.

One local FFL accepts this:

I, ________, am gifting one Sig Sauer P365 (Serial # ______) to my son, ________ (name/address).

Signed,

Name________
Address_______

Plus a copy--front AND back--of his DL.

I would contact your FFL and make sure this will suffice for them.
