My OCD would most definitely be triggered.

I had a BCM 14.5 M4’gery few years ago that was dead center on windage and elevation. Was my favorite upper with irons sights.

I say recheck the sights and rail if it’s free float. Or maybe remove the upper and place on a flat level surface to see if it’s canted or crooked.

Why not have your friend call PSA and talk to them (complain) about it.
      Maybe they will replace it....hopefully.

I think the feedback to PSA would possibly help with their quality control in the future.

I have a few that’s dead center right out the box, but then noticed a few of my other uppers the Rear would be slightly of left or right, I’ve kinda lived with it and don’t bother anymore, because IT SHOOTS lol as long I’m hitting my target I’m fine with that..

The barrel should be square with the receiver.
It is uncommon for the front face of the reciever to actually be square to the bore of the reciever.
The only receivers I routinely find to actually be square are Vltor Mur's.

3 thousandths is common on forged recievers.
Take that out 10" and you are 0.030" off.
At 14" out, you are 0.042" off.

Wow, that's quite a bit more than I'd have expected.

Do you know what the tolerance is in the TDP?

I had a friend who had a funny way of using sights so his sight picture caused him to crank all his sights over like that.

                  Maybe he needs these.

I don't have the TDP so I don't even know if they even reference one beyond the basic global tolerances.
This does not really matter when the front sight is mounted on the barrel.

It's more of a problem for sights attached to freefloat handguards.

It was cheap, so not really worth the hassle of repairing or returning.

they will say it is fine. had a PSA upper that did the same.

sent it back and they said when they tested it, there was nothing wrong.

so i've just lived with having the rear sight cranked to the right to get zero'ed.

i just mounted a 2x scope and i didn't have to crank the adjustment right. so it is probably something to do with the front sight being off. but i'm not gonna worry about it.
