I had an M1 that did this and sure enough the sear was really worn. I replaced the trigger group and have had no problems in the 32 years since.

USP 45 I bought new back in the 90's
Taking it out for the first time, I shoot a magazine through it no problem. With the slide locked back, I load a fresh mag, slam it home and the slide flies forward and blammo, it goes off.
      Finger on the trigger? Nope
Set it up again with an empty mag and slide goes forward again, but I wasn't sure if it fired so I get the rio over and explain what I think happened and we set it up again. Neither of us are sure if it "fired" so we set it up with one round in the magazine, verify no fingers on the trigger and slam the mag home.
      Blam!
Packed it up, sent it to H&K who fixed it, then I sold it off.

I have an unfired 59/66 SKS. I wonder if it would do it. The thing has been a safe Queen since the sixties.

You should see if the firing pin shakes freely in the bolt before firing it.
