Form to use to register BB AW's

Anyone know which form to use to register our so called assault weapons that have bullet buttons? I went on the CA DOJ site and found the attached but don't know if its the correct one. I'm sure there are at least three of us who will "comply" with the new BS law.
Please don't say move out of state or don't comply. I, as well as others are seriously looking for the correct form. I'm sure calling the DOJ is a waste of time, would rather see what help I can get from people on the forum.

It doesn't go into effect for a while.

When it becomes available, by law it will be a web app.

No information yet on when that might be available, beyond 'sometime in 2017'.
