This is why I tell people they need to do regular maintenance and thoroughly clean their tanks. Don't just park them for 10 years, load 'em up and go to the range. A couple more of these and they'll be added to the assault weapons ban.

Could have been caused by a couple different things.
Sobering reminder of how very important training, regular maintenance, inspection and ammunition status is in this Hobby.

What is "too soon" in this case?

There is a sticky by bwiese as a warning about hang fires and in his case it was 10-15 seconds. These type of accidents (and the sticky I linked) are very important for new shooters to give them an idea of how serious it can all get.
