In one scene, they just needed the revolver to look loaded. So instead of snap caps, some genius loaded up some rounds without powder, but they still had a live primer. At some point that round was discharged, causing a squib in the barrel.

In a later scene, the gun needed to be fired at Brandon. This time blanks were used. With the squib in the barrel, shooting a blank was almost like shooting a real round. It was a series of errors that led to his tragic death.

That whole event would have been avoided if they'd just used two matching pistols..
