Realizing that The Blaze is right-wing, let's just take the facts as listed, like Joe Friday would.
Could this possibly be a violation of the Civil Rights Act of 1964? Personally, like President Trump, I'm for the COVID-19 vaccine. But the Civil Rights Act of 1964 makes it clear that service may not be refused due to race, sex, creed, religion, and so on, including by private businesses. There's a truckload of subsequent case law that backs that up. The applicable provision here appears to be, "religion". JD Vance is now the Vice President. He has some real power now, at the Federal level. I foresee a DoJ lawsuit against that hospital, and with that precedent, others would be able to get those transplants as well.

On one hand, if you are going to get a complex "procedure" you can expect they are gonna demand you do a bunch of stuff and they ain't going to swerve from their path.
On the other hand, this is clearly just Covid power-tripping and not supported by any science, like everything about Covid.
