Tens of millions died and much of industrialized world of the time was laid to waste to get to the point of hanging the alleged perps at Nuremberg.

I'm sure plenty of ordinary German citizens asked the same questions you are OP back in the 20's and 30's as Nazism rose and medical experimentation ramped up. Ask the question too publicly and one ended up as a 'dissident' in the camps being experimented on. Hence, people kept things to themselves.

Today it's economic and legal devastation rather than outright killing dissidents in the streets or at camps. Effective though. Virus did a pretty good job too but it was an equal opportunity killer IMO.
