The "race" part is indirect. He is not wrong. If you are poor, you are more likely to have bad health care to begin with, have bad nutrition habits, and then you are more likely to work somewhere like fast food where you have direct contact with lots of people (and no paid sick leave, no way to "work from home", and no savings to rely on). Economic status absolutely is a factor to who is catching and dying of COVID. It also happens, whether directly or indirectly, that lower economic status folks are more frequently black/brown. The actual cause of that is debatable for some people, but the results of this disease are not really rocket science.

By calling it institutional racism he is is directly inferring that the plight of these poor is due to discrimination.. that it is the rest of us who are at fault for their poorness lack of education and health condition.
He should have simply said it affects the poor and unhealthy more...

Institutional racism is an excuse race baiters and white apologists fall back on when they can't explain why a large segment of the black community continues to wallow in poverty, even though they have had plenty handed to them. It's a BS term meant to excuse failure and shift blame.

Blacks and Latinos are definitely dying at a higher rate than whites from Covid, that fact is indisputable. However, they tend to be poorer with less access to quality health care and in worse health. Now, if you can control for the differences in the populations and still find minorities have a higher rate of death, that residual could constitute systemic racism. In macroeconomics there's a huge amount of literature on the technology residual, it how difficult to measure things are measured. Not my area of expertise, but I know a ton of health care disparity people from grad school and some of their arguments, even for a hard eyed data martinet like myself, are convincing.
