This, though I wouldn't outright call it operator error. Operator-caused?
If you're firing your shotgun from a bench position to sight in with slugs, etc., the slide tends to "work" itself a little given the amount of force the action endures when you "one-hand" or fire it from said bench. It's the nature of the gun.
The shell is spent so there's no risk to any nearby orphanages or hospitals.sigpic

Sounds like you "limp-wristed" it.
Similar to what happens when "limp-wristing" a pistol - which causes a double feed.

Normal. Pump guns are designed to have a hand on the pump forend when firing. When one handing, the bolt can ride back under recoil.The only thing that is worse than an idiot, is someone who argues with one.Comment

Normal and prefered if you work a pump correctly. What many guys call short stroking is that the bolt comes slightly back as you show and they bump it forward and then rack back. On a Remington (don't know about on your Mossberg) this causes a round to be release from the tube and jam between the carrier and the breech bolt. Not good.
When you learn to rack back in one smooth action you are taking advantage of the bolt coming back some and you are getting some fast action on a pump.
With recoil reducing stocks like the SpecOps or Mesa you keep your left arm loose so as not to fight the recoil reducing ablity of the stock. On these weapons you will find the breech bolt comes back almost far enough to eject the shell.
