You can also use mag disconnect as extra safety. I feel an unresponsive safety is more of liability than benefit.

Yes, return to S&W for service. Competitive bulleye shooters generally do not use the safety because forgetting to switch it off ruins a match score.Leave a comment:

Is this repeatable? Did you try to pull trigger when safety was partially engaged, and then it fired by toggling safety alone? Ruger Mark IV had a recall for this reason.

A lot of people complain of the 41's safety getting bumped to the ON position. Yours is the first I have ever heard of the gun firing merely by disengaging the safety! I think a call/email to S&W's repair shop would be in order.Leave a comment:

The safety on mine was very very difficult to engage and dis engage. So much that I simply left it dis engaged all the time. I always yelled and anyone that wanted to shoot it to keep their fingers off the trigger at all times!!Leave a comment:
