The safety on mine was very very difficult to engage and dis engage. So much that I simply left it dis engaged all the time. I always yelled and anyone that wanted to shoot it to keep their fingers off the trigger at all times!!

A lot of people complain of the 41's safety getting bumped to the ON position. Yours is the first I have ever heard of the gun firing merely by disengaging the safety! I think a call/email to S&W's repair shop would be in order.

Not an issue I have ever dealt with.

If it was me, I would call the mother ship and see what they say. My guess is they will send you a shipping label for warranty repair.

Is this repeatable? Did you try to pull trigger when safety was partially engaged, and then it fired by toggling safety alone? Ruger Mark IV had a recall for this reason.

Yes, return to S&W for service. Competitive bulleye shooters generally do not use the safety because forgetting to switch it off ruins a match score.

You can also use mag disconnect as extra safety. I feel an unresponsive safety is more of liability than benefit.
